 Case managing pregnant women in general practice leads to improved processes of care for both mother and baby.

In 2009 we developed a scheme to ensure pregnant women were case managed through their pregnancy. This initiative resulted in high rates of timeliness of immunisation of their infants. Since the inception of this ‘Best for Baby’ scheme, additional initiatives throughout the years have addressed maternal and infant health and resulted in improved processes of care.
