Focal therapy for prostate cancer: A new paradigm

Prostate cancer is the most commonly diagnosed cancer among men in New Zealand. Traditionally, curable prostate cancers have been managed through active surveillance or whole-gland treatments such as radiotherapy or radical prostatectomy. 

An emerging alternative for select patients with intermediate-risk prostate cancer is focal therapy such as Irreversible Electroporation (IRE). IRE is a minimally invasive surgical technique that involves the placement of probes within the targeted area of prostate tissue. A series of high-voltage direct current pulses is delivered between the probes, disrupting the integrity of cancer cell membranes and resulting in irreversible cell death. 

This targeted approach significantly reduces the risk of functional side effects, including urinary incontinence and erectile dysfunction, commonly associated with traditional whole-gland therapies. 

Focal therapy represents a paradigm shift in the management of intermediate-risk prostate cancer, offering effective oncological control while preserving quality of life.
